Clofibrate is an antilipidemic agent. It acts to lower elevated serum lipids and may therfore be used to treat conditions causing high cholesterol and high triglyceride levels, such as hyperlipoproteinemia type III and severe hypertriglyceridemia.

Clofibrate is a peroxisome proliferator-activated receptor alpha (PPARA) agonist in muscle, liver, and other tissues, activation of which leads to altered gene expression resulting in increased beta-oxidation, decreased triglyceride secretion, increased HDL and increased lipoprotein lipase activity.
